INFEKSI PAYUDARA PADA MASA NIFAS (MASTITIS)

A.   Definisi Infeksi Masa Nifas

Infeksi masa nifas adalah semua peradangan yang disebabkan oleh masuknya kuman-kuman kedalam alat-alat genital pada waktu persalinan dan nifas. Demam nifas atau morbiditas puerperalis meliputi demam dalam masa nifas oleh sebab apapun.

Morbiditas puerperalis ialah kenaikan suhu tubuh sampai 38˚C atau lebih selama 2 hari dalam 10 hari pertama postpartum, dengan mengecualikan hari pertama

B. Infeksi Payudara

1.      Mastitis

a.       Definisi

Dalam masa nifas dapat terjadi infeksi dan peradangan pada mammae terutama pada primipara. Tanda-tanda adanya infeksi adalah 
 rasa panas dingin disertai dengan kenaikan suhu, penderita merasa lesu dan tidak ada nafsu makan. Penyebab infeksi adalah staphilococcus aureus. Mamae membesar dan nyeri dan pada suatu tempat, kulit merah, membengkak sedikit, dan nyeri pada perabaan. Jika tidak ada pengobatan bisa terjadi abses.

b.      Penyebab

Infeksi payudara biasanya disebabkan oleh bakteri yang banyak ditemukan pada kulit yang normal (Staphylococcus aureus).Bakteri seringkali berasal dari mulut bayi dan masuk ke dalam saluran air susu melalui sobekan atau retakan di kulit (biasanya pada puting susu).Mastitis biasanya terjadi pada wanita yang menyusui dan paling sering terjadidalam waktu 1-3 bulan setelah melahirkan.Sekitar 1-3% wanita menyusui mengalami mastitis pada beberapa minggu pertama setelah melahirkan. Pada wanita pasca menopause, infeksi payudara berhubungan dengan peradangan menahun dari saluran air susu yang terletak di bawah puting susu.Perubahan hormonal di dalam tubuh wanita menyebabkan penyumbatansaluran air susu oleh sel-sel kulit yang mati. Saluran yang tersumbat inimenyebabkan payudara lebih mudah mengalami infeksi.

1. Payudara bengkak yang tidak disusukan secara adekuat.
2. Bra yang terlalu ketat.
3. Puting susu lecet yang menyebabkan infeksi.
4. Asupan gizi kurang, istirahat tidak cukup dan terjadi anemia.

c.       Gejala

1. Bengkak dan nyeri.
2. Payudara tampak merah pada keseluruhan atau di tempat tertentu.
3. Payudara terasa keras dan berbenjol-benjol.
4. Ada demam dan rasa sakit umum.

d.      Berdasarkan tempatnya infeksi dibedakan menjadi :

a)      Mastitis yang menyebabkan abses dibawah areola mamae.

b)      Mastitis ditengah-tengah mammae yang menyebabkan abses ditempat itu.

c)      Mastitis pada jaringan dibawah dorsal dari kelenjar-kelenjar yang menyebabkan abses antara mammae dan otot-otot dibawahnya.

e.        Pencegahan

Perawatan putting susu pada laktasi merupakan usaha penting untuk mencegah mastitis. Perawatan terdiri atas membersihkan putting susu dengan minyak baby oil sebelum dan sesudah menyusui untuk menghilangkan kerak dan susu yang sudah mengering. Selain itu juga memberi pertolongan kepada ibu menyusui bayinya harus bebas infeksi dengan stafilococus. Bila ada luka atau retak pada putting sebaiknya bayi jangan menyusu pada mammae yang bersangkutan, dan air susu dapat dikeluarkan dengan pijitan.

f.       Pengobatan

Segera setelah mastitis ditemukan pemberian susu pada bayi dihentikan dan diberikan pengobatan sebagai berikut :

Ø  Berikan kloksasilin 500 mg setiap 6 jam selama 10 hari.

Ø  Sangga payudara

Ø  Kompres dingin

Ø  Bila diperlukan berikan parasetamol 500 mg per oral setiap 4 jam.

Ø  Ikuti perkembangan 3 hari setelah pemberian pengobatan

Bila ada abses, nanah perlu dikeluarkan dengan sayatan sedikit mungkin pada abses, dan nanah dikeluarkan sesudah itu dipasang pipa ketengah abses, agar nanah bisa keluar. Untuk mencegah kerusakan pada duktus laktiferus sayatan dibuat sejajar dengan jalannya duktus-duktus. Atau jika terdapat masa padat, mengeras dibawah kulit yang kemerahan :

Ø  Berikan antibiotik kloksasilin 500 mg per oral 4 kali sehari selama 10 hari atau eritromisin 250 mg per oral 3 kali sehari selama 10 hari

Ø  Drain abses :

o Anestesi umum dianjurkan

o Lakukan insisi radial dari batas puting ke lateral untuk menghindari cidera atau duktus

o Gunakan sarung tangan steril

o Tampon longgar dengan kasa

o Lepaskan tampon 24 jam ganti dengan tampon kecil

Ø  Jika masih banyak pus tetap berikan tampon dalam lubang dan buka tepinya

Ø  Yakinkan ibu tetap menggunakan kutang

Ø  Berikan paracetamol 500 mg bila perlu

Ø  Evaluasi 3 hari

g.      Penangan Dan Peran Bidan

1. Payudara dikompres dengan air hangat.
2. Untuk mengurangi rasa sakit dapat diberikan pengobatan analgetika.
3. Untuk mengatasi infeksi diberikan antibiotika.
4. Bayi mulai menyusu dari payudara yang mengalami peradangan.
5. Anjurkan ibu selalu menyusui bayinya. fadlie.web.id
6. Anjurkan ibu untuk mengkonsumsi makanan yang bergizi dan istirahat cukup.

7.      Konseling suportif
            Mastitis merupakan pengalaman yang sangat nyeri dan membuat frustrasi, dan membuat banyak wanita merasa sangat sakit. Selain dengan penanganan yang efektif dan pengendalian nyeri, wanita membutuhkan dukungan emosional. Ibu harus diyakinkan kembali tentang nilai menyusui; yang aman untuk diteruskan; bahwa ASI dari payudara yang terkena tidak akan membahayakan bayinya; dan bahwa payudaranya akan pulih baik bentuk maupun fungsinya.

8.      Pengeluaran Asi Dengan Efektif

Dengan membantu ibu memperbaiki kenyutan bayi pada payudara, mendorong untuk sering menyusui, sesering dan selama bayi menghendaki, tanpa pembatasan, bila perlu peras ASI dengan tangan atau dengan pompa atau botol panas, sampai menyusui dapat dimulai lagi.         

2.      Bendungan ASI

 a. Definisi

Bendungan ASI adalah pembendungan air susu karena penyempitan duktus laktiferi atau oleh kelenjar yang tidak dikosongkan dengan sempurna atau karena kelainan pada putting susu (Mochtar, 1996).

Menurut Huliana (2003) payudara bengkak terjadi karena hambatan aliran darah vena atau saluran kelenjar getah bening akibat ASI terkumpul dalam payudara. Kejadian ini timbul karena produksi yang berlebihan, sementara kebutuhan bayi pada hari pertama lahir masih sedikit.

b. Patologi

Faktor predisposisi terjadinya bendungan ASI antara lain :

 Faktor hormon

 Hisapan bayi

 Pengosongan payudara

 Cara menyusui

 Faktor gizi

 Kelainan pada puting susu

c.  Patofisiologi

a.       Gejala yang biasa terjadi pada bendungan ASI antara lain payudara penuh terasa panas, berat dan keras, terlihat mengkilat meski tidak kemerahan.

b.      ASI biasanya mengalir tidak lancar, namun ada pula payudara yang terbendung membesar, membengkak dan sangat nyeri, puting susu teregang menjadi rata.

c.       ASI tidak mengalir dengan mudah dan bayi sulit mengenyut untuk menghisap ASI. Ibu kadang-kadang menjadi demam, tapi biasanya akan hilang dalam 24 jam (Mochtar, 1998).

d.      Penatalaksanaan dan Peran Bidan

Upaya pencegahan untuk bendungan ASI adalah :

 Menyusui dini, susui bayi sesegera mungkin (setelah 30 menit) setelah dilahirkan

 Susui bayi tanpa jadwal atau ondemand

 Keluarkan ASI dengan tangan atau pompa, bila produksi melebihi kebutuhan bayi

 Perawatan payudara pasca persalinan

Upaya pengobatan untuk bendungan ASI adalah :

d.      Kompres hangat payudara agar menjadi lebih lembek

e.       Keluarkan sedikit ASI sehingga puting lebih mudah ditangkap dan dihisap oleh bayi.

f.       Sesudah bayi kenyang keluarkan sisa ASI

g.      Untuk mengurangi rasa sakit pada payudara, berikan kompres dingin

h.      Untuk mengurangi statis di vena dan pembuluh getah bening lakukan pengurutan (masase) payudara yang dimulai dari putin kearah korpus. (Sastrawinata, 2004)

3.      Abses Payudara

a.       Definisi

Abses payudara berbeda dengan mastitis. Abses payudara terjadi apabila mastitis tidak tertangani dengan baik, sehingga memperberat infeksi.

b.      Gejala

1. Sakit pada payudara ibu tampak lebih parah.
2. Payudara lebih mengkilap dan berwarna merah.
3. Benjolan terasa lunak karena berisi nanah.
4. Payudara yang tegang dan padat kemerahan.
5. Pembengkakan dengan adanya fluktuasi.
6. Adanya pus/nanah.

c.       Penanganan dan Peran Bidan

1. Teknik menyusui yang benar.
2. Kompres payudara dengan air hangat dan air dingin secara bergantian.
3. Meskipun dalam keadaan mastitis, harus sering menyusui bayinya.
4. Mulailah menyusui pada payudara yang sehat.
5. Hentikan menyusui pada payudara yang mengalami abses, tetapi ASI harus tetap dikeluarkan.
6. Apabila abses bertambah parah dan mengeluarkan nanah, berikan antibiotik.
7. Rujuk apabila keadaan tidak membaik.

4.      Saluran tersumbat

Saluran tersumbat hampir selalu dapat terselesaikan tanpa pengobatan khusus antara 24 hingga 48 jam setelah terjadi. Selama sumbatan itu masih ada, bayi mungkin saja rewel ketika menyusu di payudara tersebut karena aliran ASI akan lebih lambat dari biasanya. Hal ini mungkin disebabkan karena adanya tekanan dari benjolan yang menekan saluran lain. Saluran tersumbat dapat diatasi lebih cepat jika :

1)      Teruskan menyusui pada payudara yang sakit, dan kosongkan payudara dengan lebih baik. Hal ini dapat dilakukan dengan :

a.       Sedapat mungkin melakukan pelekatan yang baik (lihat lembar informasi “Ketika Melekat/When Lacthing” juga video klip bagaimana melekatkan bayi pada situs nbci.ca)

b.      Menggunakan tekanan pada payudara untuk menjaga ASI tetap mengalir (lihat lampiran informasi “Penekanan Payudara/Breast Compression” dan video klip bagaimana melekatkan bayi pada situs nbci.ca). Letakkan tangan di sekitar saluran yang tersumbat dan jika tidak terlalu sakit, tekan saat bayi sedang menyusui.

c.       Susui bayi dengan posisi sedemikian rupa sehingga dagu bayi ”mengarah” pada saluran yang tersumbat. Jadi, bila saluran tersumbat ada pada bagian luar bawah payudara (arah jam 7), maka menyusui bayi dengan posisi football dapat sangat membantu.

2)      Hangatkan area yang terinfeksi. Anda bisa melakukan ini dengan bantalan penghangat atau botol berisi air panas, tetapi hati-hati untuk tidak membakar kulit dengan menempelkan yang terlalu panas untuk waktu yang terlalu lama.

3)      Coba untuk beristrirahat. Tentu saja, dengan kehadiran seorang bayi baru tidaklah mudah untuk beristirahat. Cobalah untuk tidur. Bawa bayi bersama Anda ke tempat tidur dan susui dia di sana.
